Just call us after checkout to choose your appointment time.

2. Connect with your insurance agency
You can call the phone number on the back of your insurance card and/or access your account portal through your insurance agency’s website – we personally recommend going online first!
3. Follow the prompts to see your benefits and eligibility info
You might have to make or activate your account online. If you call in, you’ll have to share your policy information (likely with an automated system). Once this is done, find your way to your Benefits and Eligibility.
- For exam benefits: You’ll want to look up the following CPT codes: 99202; 99203; 99204; 99212; 99213; and 99214. You will want to look for how many visits are covered this year and to what extent – there will likely be a copay or a co-insurance! Don’t forget to check if it applies to your deductible!
- For chiropractic benefits: You’ll want to look up the following CPT codes: 98940; 98941; 98942; and 98943. You will want to look for how many visits are covered this year and to what extent – there will likely be a copay or a co-insurance! Don’t forget to check if it applies to your deductible!
- For additional benefits (including massage): You’ll want to look at the following CPT codes: 97110; 97112; 97124; 97140; and 97535. Codes 97124 and 97140 are for massage, and you will need to specify that this service will be provided by a Licensed Massage Therapist (LMT). Some insurance plans will say that you have massage benefits… but they don’t tell you that it doesn’t cover services provided by an LMT!
